Recent Board Actions (March 3-4, 2016)

At the March 8 & 9, 2016 meeting of the Iowa Board of Pharmacy (Board), the following public actions were taken:

Drug Depot, Nonresident Pharmacy, entered into a Settlement Agreement and Final Order in which it was cited and warned for providing pharmacy services to Iowa residents without an active Iowa pharmacy license and agreed to pay a civil penalty of $2,500.  The Settlement Agreement resolves the Statement of Charges filed on January 12, 2016. 

Pharmacy Creations, Nonresident Pharmacy, entered into a Combined Amended Statement of Charges, Settlement Agreement and Final Order in which it was cited and warned for violating pharmacy laws or rules of another state and for failing to timely report discipline.  Respondent also agreed to pay a civil penalty of $2,500. 

Wickliffe Veterinary Pharmacy, Nonresident Pharmacy, entered into a Combined Statement of Charges, Settlement Agreement and Final Order, which charged Respondent with violating the pharmacy laws or rules of another state and placed its pharmacy license on probation for the duration of its probationary period with the Kentucky Board of Pharmacy.  Respondent also agreed to pay a civil penalty of $1,000.

Kelsey Troxel, Technician Trainee, entered into a Combined Statement of Charges, Settlement Agreement and Final Order in which Respondent voluntarily surrendered her pharmacy technician trainee registration for a charge of diverting prescription drugs.

David Schlueter, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement and Final Order to indefinitely suspend his pharmacist license until he completes an evaluation, completes continuing education hours, and pays a civil penalty of $1,000 for a charge of forging prescriptions.

Jerod Work, Pharmacist.   The Board filed a Notice of Hearing and Statement of Charges, charging the pharmacist with unprofessional behavior by theft, failure to record purchases of pseudoephedrine, and failure to notify of change in employment.  A hearing is scheduled for May 3, 2016. 

Angela Buckland, Technician Trainee.  The Board filed a Notice of Hearing and Statement of Charges, charging the technician with diversion of drugs and conviction of a crime.  A hearing is scheduled for May 3, 2016.
