Recent Board Actions (May 3-4, 2016)

At the May 3 - 4, 2016 meeting of the Iowa Board of Pharmacy (Board), the following action was taken:

Inie Clement, Pharmacist. The Board granted the petition to vacate the Findings of Fact, Conclusions of Law, Decision and Order issued January 16, 2016. A new hearing on the June 23, 2015 Statement of Charges is scheduled for June 29, 2016.

Melanie Howard, ARNP. The Board filed an Order to Show Cause for her controlled substance act registration.

Lisa Gray, Pharmacist, entered into a Settlement Agreement and Final Order that included a Citation and Warning, civil penalty, and a requirement to complete continuing education. This resolves the Statement of Charges filed on November 4, 2015.

Angela Buckland, Technician entered into a Settlement Agreement and Final Order to VOLUNTARILY SURRENDER her pharmacy technician registration to resolve the Statement of Charges filed on March 8, 2016.

Alpine Wellness, Inc. Wholesale Drug License entered into a Settlement Agreement and Final Order, in which Respondent agreed to a permanent VOLUNTARILY SURRENDER its Iowa wholesale drug license. This resolves the Statement of Charges filed on October 23, 2015.

Amy Hanen, Technician. The Board filed a Notice of Hearing and Statement of Charges charging the technician with diversion of drugs. A hearing is scheduled for June 29, 2016.

Wendell Simmonds, Pharmacist. The Board filed a Notice of Hearing and Statement of Charges charging the pharmacist with failure to properly verify a prescription. A hearing is scheduled for June 29, 2016.

Pharmacy Matters, Pharmacy. The Board filed a Notice of Hearing and Statement of Charges, charging the pharmacy with failure to maintain continuous quality improvement program, failure to have sufficient policies and procedures and unethical behavior. A hearing is scheduled for June 29, 2016.

Mike Stein, Pharmacist. The Board filed a Notice of Hearing and Statement of Charges, charging the pharmacist with violating the responsibilities of a pharmacist-in-charge. A hearing is scheduled for June 29, 2016.

Greenville Pharmacy, Pharmacy. The Board filed a Notice of Hearing and Statement of Charges, charging the pharmacy with fraud in procuring a license. A hearing is scheduled for June 29, 2016.

Robert P. Rehal, Pharmacist. The Board filed a Notice of Hearing and Statement of Charges, charging the pharmacist with fraud in procuring a license. A hearing is scheduled for June 29, 2016.

Edita Husic, Technician. The Board approved a Combined Statement of Charges, Settlement Agreement and Final Order, in which Respondent voluntarily surrendered her pharmacy technician trainee registration to resolve a charge of diverting drugs.

Rockford Anderson, Pharmacist. Entered into a Settlement Agreement and Final Order that included a citation and warning, a civil penalty, and a requirement to complete continuing education. This resolves the Statement of Charges filed on January 12, 2016.

Ankeny Pharmacy, Pharmacy. Entered into a Settlement Agreement and Final Order that placed the pharmacy’s license on probation for four years with requirements audits and self-inspections and required payment of a civil penalty. This resolves the Statement of Charges filed on January 12, 2016.
