The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held September 10, 2019:
Michael Brixius, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board. The Board issued a citation and warning for employing a technician without a current registration, failing to comply with controlled substance inventory rules, failing to reconcile Schedule II perpetual inventory, and failing to properly maintain supplier invoices. The Board further ordered a civil penalty of $1000.00 and submission of an essay.
Foodland Pharmacy, located in Woodbine, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board. The Board issued a citation and warning for failing to comply with rules governing pharmacy closing. The Board further ordered a civil penalty of $3500.00.
Walgreens 359, located in Des Moines, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order with the Board. The Board issued a citation and warning for failing to provide counseling. The Board further ordered a civil penalty of $1500.00
Jacob Taylor, Certified Technician. The Board issued a Notice of Hearing and Statement of Charges against Technician Taylor, charging him with unethical behavior. A hearing is scheduled for November 5, 2019.
Wells Pharmacy Network LLC, located in Ocala, Florida. The Board issued a Notice of Hearing and Statement of Charges against Wells Pharmacy Network LLC, charging the nonresident pharmacy with violation of a Board Order and compounding essentially copies of approved drugs. A hearing is scheduled for November 5, 2019
Margaret Bowen, Pharmacist. The Board issued a Notice of Hearing and Statement of Charges against Pharmacist Bowen, charging her with failing to comply with IMP3 contract. A hearing is scheduled for November 5, 2019.
