Recent Board Action (November 5, 2019)
The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held November 5, 2019:
Andrea L Venteicher, MD, CSA Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Order charged her with having disciplinary action taken against her medical license and restricts her CSA registration in a manner that mirrors the prohibition on prescribing, administering, or dispensing controlled substances for the treatment of chronic pain imposed by the Iowa Board of Medicine.
Stephanie Olson, DVM, CSA Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Order charged her with obtaining, possessing, or attempting to obtain or possess prescription drugs without lawful authority and for failing to change the address of the registered location. Dr. Olson voluntarily surrendered her CSA registration to resolve the disciplinary charges.
Guaranteed Returns, Limited Distributor and CSA Registrant, located in Holbrook, New York, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Order charged the limited distributor with conviction of a crime related to the distribution of prescription drugs and controlled substances and resolved the charges by staying a two year suspension of the limited distributor license and CSA registration and placing the limited distributor on probation for a period of two years with several terms and imposing a civil penalty in the amount of $25,000.00.
Jacob Taylor, Certified Technician, entered into a Settlement Agreement and Final Order with the Board. Technician Taylor voluntarily surrendered his certified technician registration to resolve the disciplinary charge for unethical behavior.
Shayna Werner, Certified Technician, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Order charged her with working without a current registration. The Board issued a citation and warning for working in a pharmacy with an expired registration. The Board further ordered a civil penalty of $100.00.
Amanda Haugen, Certified Technician. The Board issued a Notice of Hearing and Statement of Charges against Technician Haugen, charging her with unethical behavior. A hearing is scheduled for January 7, 2020.
