Recent Board Action (February 25-26, 2020)

The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held February 25-26, 2020.

Wells Pharmacy Network, LLC, located in Ocala, FL, entered into a Settlement Agreement and Final Order.  The Order charged the nonresident pharmacy with violation of a Board Order and compounding essentially copies of approved drugs.  The Board issued a stayed suspension and placed the nonresident pharmacy license and CSA registration on probation for a period of two years. 

Jordan (Manuel), Chelsea, Certified Technician, voluntarily surrendered her certified technician registration due to allegations of diversion of controlled substances.

Gates, Heather, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Board charged Pharmacist Gates with unprofessional conduct, specifically theft.  The Board issues a citation and warning for engaging in unprofessional conduct or behavior.  The Board further ordered a civil penalty of $1,000.00.

Haugen, Amanda, Certified Technician.  The Board issued a Findings of Fact, Conclusions of Law, Decision, and Order. The Board determined Technician Haugen engaged in unprofessional conduct, specifically theft.  The Board issues a citation and warning for engaging in unprofessional conduct or behavior.  The Board further ordered a civil penalty of $1,000.00.

Simonton, Kathy, Certified Technician.  The Board issued a Findings of Fact, Conclusions of Law, Decision, and Order.  The Board denied Technician Simonton’s request for reinstatement of her certified technician registration, and permitted her to reapply six months from the date of the Order.

Boca Pharmacy, Applicant, located in Bronx, NY.  The Board denied the license application for Boca Pharmacy due to having a license revoked or suspended or having other disciplinary action taken by a licensing authority of this state or another state territory, or country for conduct substantially equivalent to any of the grounds for disciplinary action in Iowa.

Sanders, Debra, Pharmacy Support Person Applicant.  The Board denied the pharmacy support person application for Ms. Sanders for violation of this state relating to prescription drugs, controlled substances, or nonprescription drugs.
