Recent Board Action (June 23, 2020)

The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held June 23, 2020

Jacob Alanis, Pharmacist-intern Applicant, Mount Pleasant.  The Board issued a Notice of Intent to Deny Pharmacist-Intern Registration due to the applicant’s criminal history.

Patrick Kin-Yee Chau, MD, CSAR Applicant, Sioux City.  The Board issued Dr. Chau’s CSA registration by Consent Agreement.  The Board issued a Citation and Warning for failing to provide truthful and accurate information on his application for CSA registration and imposed a civil penalty of $1,000.00. Price Chopper, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with failing to maintain effective controls against theft of controlled substances.  The Board issued a citation and warning and imposed a civil penalty of $10,000.00.

Courtney Martin, CPhT, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged her with failing to maintain a current technician registration with the Board while being employed as a technician.  The Board issued a citation and warning and imposed a civil penalty of $100.00.

Anthony Pape, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged him with diverting prescription drugs from a pharmacy for personal use and engaging in practice harmful or detrimental to the public.  The Board indefinitely suspended Pharmacist Pape’s license and established a probationary period when his license is reinstated. 

Walgreens 05886, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with failing to maintain effective controls against theft of controlled substances and failure to keep an accurate perpetual inventory.  The Board placed the pharmacy on probation for three years and imposed a civil penalty of $2,000.00.

Hy-Vee Pharmacy 1235, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with employing a pharmacy technician without a current registration.  The Board issued a citation and warning and imposed a civil penalty of $3,500.00.  

Price Chopper, Des Moines, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with failing to maintain effective controls against theft of controlled substances.  The Board issued a citation and warning and imposed a civil penalty of $10,000.00.
