Recent Board Action (August 26, 2020)

The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held August 26, 2020

Cori Johnson, PSP.  The Board issued a Notice of Hearing and Statement of Charges against PSP Johnson, charging her with failing to maintain a current pharmacy support person registration with the Board while being employed as a pharmacy support person. 

Lewis Family Drug #62, Pharmacy, located in Rock Rapids, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with employing a pharmacist without a current and active pharmacist license.  The Board issued a citation and warning and imposed a civil penalty of $5,000.00.

Lisa Doorenbos, Pharmacist, entered into a Combined Statement of Charges, Settlement Agreement and Final Order.  The Board charged her with practicing pharmacy without a current pharmacist license and failure to complete required continuing pharmacy education.  The Board issued a citation and warning, imposed a civil penalty of $1,000.00, and required completion of 70 hours of continuing education. 

Katie Ingalls, CPhT, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged her with failing to maintain a current technician registration with the board while being employed as a technician. The Board issued a citation and warning and imposed a civil penalty of $25.00.

Nicole Darrington, PSP, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged her with failing to maintain a current pharmacy support registration with the board while being employed as a pharmacy support person. The Board issued a citation and warning and imposed a civil penalty of $50.00.

Heather Crabtree, PSP, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged her with failing to maintain a current pharmacy support registration with the board while being employed as a pharmacy support person. The Board issued a citation and warning and imposed a civil penalty of $50.00.

Ted George, DO, CSAR Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged him with having his professional license disciplined and restricted his CSA registration in a manner that mirrors the prohibition on prescribing, administering, or dispensing controlled substances for the treatment of chronic pain imposed by the Iowa Board of Medicine.   

Christopher Rothe, PSP, voluntarily surrendered his pharmacy support person registration due to allegations of diversion of controlled substances.

Jerrett Singleton, Pharmacy Support Person Applicant.  The Board issued a Notice of Intent to Deny Registration due to the applicant’s criminal history and failure to disclose criminal history .

Woodland Hills Pharmacy, Nonresident pharmacy Applicant, Woodland Hills, CA.  The Board issued a Notice of Intent to Deny License due to the applicant’s discipline history.
