The COVID-19 Vaccination Program Provider Agreement is now available. Iowa health care providers who intend to provide COVID-19 vaccines need to complete the COVID-19 Pandemic Vaccination Provider Agreement via a REDCap survey. The attached letter provides a link to the REDCap survey and information regarding IRIS enrollment. Additionally, attached is a Frequently Asked Questions (FAQ) document for assistance in completing the COVID-19 Pandemic Vaccination Provider Agreement. Please distribute these resources to your membership and partners.
