Recent Board Action (December 16, 2020)

The Iowa Board of Pharmacy (Board) took the following actions at its most recent meeting held December 16, 2020.

Gregory Judas, PSP.  The Board issued his PSP registration by Consent Agreement, which requires him to notify any current or future pharmacy employers of the Agreement.

Surmasis Pharmaceutical, Limited Distributor, Des Moines.  The Board issued its Limited Distributor license by Consent Agreement.  The Board issued a citation and warning and imposed a civil penalty of $1,000.00 for shipping prescription drugs without a current license.

Julie Lange, CPhT.  The Board issued a Combined Notice of Hearing, Statement of Charges, and Emergency Adjudicative Order against Technician Lange, charging her with failure to comply with IMP3 contract and indefinitely suspending her CPhT registration.  A hearing is scheduled for January 12, 2021.

Hannah Rindels, CPhT, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged her with unethical conduct.  The Board issued a citation and warning, imposed a civil penalty of $100.00, and required her to participate in IMP3.

Derek Kerr, DDS, CSAR Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged him with failing to maintain a CSA registration.  The Board issued a citation and warning and imposed a civil penalty of $500.00.

Chadwick Johnson, DDS, CSAR Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged him with having disciplinary action taken against his dental license. The Board issued a citation and warning, imposed a civil penalty of $1000.00 and placed his CSA registration on probation to run concurrent with the probation on his dental license.
