The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held January 12, 2021.

Karen Potaczek, DDS, CSAR Registrant, entered into a Combined Statement Charges, Settlement Agreement, and Final Order. The Board charged her with failing to maintain a CSA registration and for deficiencies found during an on-site inspection.  The Board placed her CSA registration on probation for a period of one year and imposed a civil penalty of $3,000.00. 

Lauren Hughes, DVM, CSAR Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged her with failing to maintain a CSA registration.  The Board issued a citation and warning and imposed a civil penalty of $500.00

Julie Lange, CPhT, entered into a Settlement Agreement and Final Order. The Board charged her with failure to comply with IMP3 contract and indefinitely suspended her CPhT registration. In the Order, Ms. Lange agreed to voluntarily surrender her CPhT registration.

Walgreens #11153, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with failing to timely file a PIC change application.  The Board issued a citation and warning and imposed a civil penalty of $1,000.00.

Walgreens #02445, Orlando, FL, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with contributing to a dispensing error.  The Board issued a citation and warning, imposed a civil penalty of $3,000.00, and required the pharmacy to submit a corrective action plan to reduce the occurrence of dispensing errors.

Walgreens #07967, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.  The Board charged the pharmacy with dispensing an incorrect prescription.  The Board issued a citation and warning, imposed a civil penalty of $3,000.00, and required the pharmacy to submit a corrective action plan to reduce the occurrence of dispensing errors. 

Marlene McNaughton, CPhT.  The Board issued a Notice of Hearing and Statement of Charges, charging her with unethical conduct.  A hearing is scheduled for March 9, 2021.

Donna Rizzo, DVM, CSAR Registrant.  The Board issued a Notice of Hearing and Statement of Charges, charging her with lacking policies and procedures for handling controlled substances, failure to maintain required records pertaining to controlled substances, failure to account for several controlled substances, inconsistent information regarding the disposition of controlled substances, failure to sign and date invoices for receipt of controlled substances, failure to reconcile perpetual inventory of controlled substances and lacked documentation of incident reports, failure to complete annual inventories, failure to maintain destruction records, failure to record administration waste, returned dispensed prescription to stock, dispensed expired controlled substances, and failure to disclose disciplinary actions. A hearing is scheduled for March 9, 2021.
