Recent Board Action (March 9-10, 2021)

The Iowa Board of Pharmacy (“Board”) took the following actions at its most recent meeting held March 9-10, 2021.

James Kaufman, RPh, entered into a Reinstatement by Consent Agreement with the Board. The Agreement requires him to pass both components of the licensure examination and complete 800 hours of internship to reactivate his pharmacist license.  The Agreement places his pharmacist intern registration and pharmacist license, when issued, on probation for 3 years subject to monitoring terms.

United Community Services, CSAR Registrant, Fort Dodge and Ames, entered into a Combined Statement of Charges, Settlement Agreement and Final Order. The Board charged United Community Services with failure to timely report loss and failure to maintain accountability of controlled substances and records. The Board imposed a civil penalty of $3,000 and placed the CSA registrations on probation for 18 months subject to monitoring terms.

Glenn Young, DDS, CSAR Registrant, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Board charged him with failure to maintain a CSA registration. The Board issued a citation and warning and imposed a civil penalty of $500.

Kelly Malmberg, CPhT, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Board charged her with failure to maintain national certification. The Board issued a citation and warning and imposed a civil penalty of $100.

Donna Rizzo, DVM, CSAR Registrant, entered into a Settlement Agreement and Final Order. She agreed to voluntarily surrender her CSA registration. 

Hy-Vee Pharmacy #1573, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order. The Board charged the pharmacy with failing to ensure proper registration of pharmacist-intern. The Board issued a citation and warning and imposed a civil penalty of $1,000.
