The Iowa Board of Pharmacy (“Board”) took the following actions at its more recent meeting held May 2, 2023.
Sarah Taylor, CPhT, Council Bluffs. The Board issued a Notice of Hearing, Statement of Charges, and Emergency Adjudicative Order. A hearing is scheduled for June 27, 2023.
Walgreens 05721, Des Moines, entered into a Combined Statement of Charges, Settlement Agreement, and Final Order.
Thien-Lam Pham, Pharmacy Support Person, Clive, entered into a Settlement Agreement and Final Order.
Gabriel Podraza, RPh, Pharmacist Reactivation Applicant. The Board approved Mr. Podraza’s pharmacist license reactivation by Consent Agreement.
