Pharmacy Reporting

All Iowa licensed pharmacies, whether the pharmacy is located within or outside the state of Iowa, are required to report to the Iowa PMP. Reports are to be submitted weekly. For purposes of reporting to the Iowa PMP, a week is Sunday through Saturday and reports must be submitted to the Iowa PMP no later than Wednesday following the end of a weekly reporting period.

In-state pharmacies are required to report all Schedule II, III, and IV controlled substance prescriptions dispensed to patients regardless of the location of the patient.

Nonresident pharmacies are required to report all Schedule II, III, and IV controlled substance prescriptions dispensed to patients physically located in Iowa.

A pharmacy that does not dispense Schedule II, III, or IV controlled substances as provided above, or that believes the pharmacy should be exempt from reporting to the Iowa PMP as provided by law (i.e. a pharmacy dispensing only for inpatient hospital care, inpatient hospice care, or long-term residential facility patient care), may request exemption from reporting to the Iowa PMP. To request exemption, fill out the pharmacy request for exemption from reporting controlled substances prescriptions form and return to the Iowa Board of Pharmacy.

The PMP Data Collection Manual includes information regarding reporting to the Iowa PMP and provides contact information for assistance with establishing a reporting login account and submitting weekly reports.
